You've made a step in the right direction to setting up your OneRecord and managing your health records but it looks like we weren't able to connect your portal. We're sorry for this experience.

The top four reasons are:

  1. You are not registered with your portal.

  2. Your user name and password combination for your portal are incorrect.

  3. The portals' authorization server is down.

  4. More technical reasons that we won't bore you with.
